MX Helps Hospitals Log Discharges for Patients Experiencing Homelessness

The Senate Bill (SB) 1152 hospital patient discharge process includes provisions intended to help patients experiencing homelessness prepare for return to the community by connecting them with available community resources. Requirements for hospitals include the creation of a log of discharged patients experiencing homelessness and documentation of compliance with various other measures. Manifest MedEx can help hospitals meet this requirement with an automated monthly audit report that documents the admission and discharge of patients identified as experiencing homelessness. New Risk Scores for MX Analyze

MX is adding risk scores to the longitudinal health records available in MX Access to help users better understand the risk profiles of patients for whom they’re managing and providing care.

Users can now see the first 2 risk scores below in MX Access, and will be able to see the third risk score later this month. These risk scores come from MX Analyze and are generated by our partner HBI Analytics using the claims and clinical data in MX’s system for the patient.

1. IP Admission in the Future 12 Months: The probability the patient will have an inpatient admission within the next 12 months
2. ED Visit Risk in the Future 12 Months: The probability the patient will have an emergency department visit within the next 12 months
3. COVID-19 Related Mortality: The risk of death during a COVID-19 episode of illness if the patient is infected.

MX has also integrated the following 5 new risk scores in MX Access for IEHP members

1. Probability of High Total Cost: The probability that the member will have costs in the top 5% of the population in the next 12 months.
2. Probability of Persistent High User: The probability that the member will be in the top 20% of users for 2 consecutive years.
3. Risk Score: The risk associated with the individual member compared to 1, where 1 is defined as an average risk based off of a large external reference population.
4. Chronic Condition Count: The number of chronic conditions identified for the member.
5. Care Management Risk Stratification: Risk Stratification of IEHP Membership based on Care Coordination, Diagnosis Based Markers and Risk Modeling.

Sending Real-Time Critical Information to Emergency Departments

On December 4th, MX launched a new Alerts Viewer as part of the +EMS program to send critical information real-time on inbound patients to emergency departments. The Alerts Viewer is currently being utilized by 9 hospitals. +EMS SAFR is currently being implemented in 5 counties across California. MX is the first +EMS grantee to complete Milestone 1-b in the state across any active +EMS grants with Cal EMSA, and has been granted two expansion awards for the grant to include more hospitals and portable medical orders (POLST).
