2023: An Exciting Year of Impact and Transformation
for Manifest MedEx and California
2023 has been another exciting and transformative year at Manifest MedEx as we continue to advance the equitable exchange of health information across California in support of our state’s ambitious goals to address health equity and build a healthy California for all.
Among the highlights this year:
- Manifest MedEx was designated one of nine CalHHS Data Exchange Framework (DxF) Qualified Health Information Organizations (QHIOs). We look forward to continuing to support all DxF participants, regardless of their location, technical capabilities, or resources so that we can truly advance data sharing to ensure seamless, coordinated care for all Californians.
- Manifest MedEx successfully implemented the new data system for the California Department of Public Health’s Surveillance and Public Health Information Reporting and Exchange (SaPHIRE). We migrated electronic lab results and data feeds from more than 400 labs and other entities that submit lab results to the California Department of Public Health (CDPH) and continue to connect labs across the state. Today more than 1500 labs participate in electronic lab reporting through SaPHIRE.
- In addition to electronic lab reporting, Manifest MedEx now provides electronic case reporting to CDPH, providing more than 65,000 CCDAs per month to help support public health efforts.
We were selected to participate in DHCS’ Authorization to Share Confidential Medi-Cal Information (ASCMI) Pilot in collaboration with San Joaquin County, Health Plan of San Joaquin, and Health Net. Consent management is critical to the success of CalAIM, which depends on seamless care coordination across physical health, behavioral health, and social service providers. - Thanks to our participants, public health officials, and health policy leaders who make health data exchange possible, today, we securely share permitted health data for approximately 38 million Californians — over 90% of the state’s population — and deliver 1.5 million admission, discharge, and transfer (ADT) notifications per month across a network of more than 1,800 healthcare providers, 125 hospitals, and 17 health plans, including Aetna, Blue Shield of California, Anthem Blue Cross, Health Net, Health Plan of Sam Joaquin, and Inland Empire Health Plan.
- This year, we increased portal users by 30 percent, had 127% more patients subscribed in MX Notify, and expanded individuals with a health record in our network by almost 10 percent, helping improve patient experience and outcomes across the state.
- In response to participant feedback and as part of our commitment to support the DxF, we launched significant enhancements to our tools and products that will help streamline performance, save participant time, and provide a better user experience. This includes enhancements in MX Notify and MX Access, as well as bringing in more than 500,000 clinical care summaries from Kaiser Permanente California, CommonSpirit Health, Sutter Health, UC Health System, MemorialCare, Cedars-Sinai, and other California health systems when we know an episode of care is occurring at a hospital on the MX network, by facilitating treatment-driven exchange through our participation with national networks like eHealth Exchange and Carequality.
- We are in the process of implementing additional enhancements that will better support meeting our QHIO DxF requirements, including enabling secure, electronic individual access (see below) so that patients have easy and timely access to their health data to support their own health and wellbeing.
- Manifest MedEx also earned NCQA Data Aggregator Validation Designation again (see below), marking the fourth consecutive year that health plans and providers can rely on validated data from Manifest MedEx as supplemental data for HEDIS® quality reporting.
- Manifest MedEx co-hosted the 2023 Annual California Data Exchange Framework (DxF) Summit themed, “Cultivating Connections: Expanding Health and Human Services Data Exchange to Advance Health Equity!” with the California Health and Human Services’ Center for Data Insights and Innovation, California Health Care Foundation, and Connecting for Better Health. During this year’s summit, we focused on bold and honest conversations about the DxF and progress to implement, health data policies and their impact in our state, and how various initiatives that involve health and human services exchange are aligning or running parallel.
- MX's platform has again successfully met the requirements of HITRUST CSF® certification, demonstrating the highest levels of security in the healthcare industry. Manifest MedEx has consistently earned HITRUST CSF® Certification since 2019.
As we come to the end of the year, we thank our participants for their partnership in making these advancements possible and for their continued support in expanding health data exchange across California to enable digital equity that advances public health, Medi-Cal priorities, and health equity. Manifest MedEx to Enable Secure, Electronic Patient Access in 2024
At Manifest MedEx, we believe that patient data belongs to the patient, and patients should have easy and timely electronic access to their health records to support their own health and wellbeing. Having access to health records enables patients to better coordinate care with their providers and health plans and ensure the accuracy of information captured in health records.
To facilitate individual access to records in our network, we are excited to announce we will be launching a first-in-class, secure patient identity verification and electronic records access service in 2024. Enabling this functionality is also a requirement for Manifest MedEx to serve as a designated qualified health information organization (QHIO) – one that we enthusiastically support.

MX Earns NCQA Validated Data Stream Designation For a Fourth Consecutive Year!
Manifest MedEx has earned the Validated Data Stream designation in the National Committee for Quality Assurance (NCQA) Data Aggregator Validation program for a fourth consecutive year. Seen as a vital, early step in achieving NCQA's mission of a digital measurement ecosystem, validated data flows improve the efficiency of today's quality measurement processes. Health plans and providers can utilize Manifest MedEx to optimize their HEDIS® reporting process using both validated data and HEDIS® data extracts.

Participant Bright Spot
San Joaquin County Public Health Services (SJCPHS) aims to advance health equity, promote healthy living, and improve the conditions that impact the overall well-being of over 790,000 residents in San Joaquin County. Located in Central California, San Joaquin County (SJC) is among the fastest growing counties in the state.
In a study published by UC Davis in June 2022, SJC was one of the three counties with significantly higher rates of congenital syphilis (CS) compared to larger metro counties, with 297 CS cases per 100,000 live births. This study correlated higher CS cases to high poverty rates, identifying barriers of associated burdens of out-of-pocket costs, employment demands, and housing insecurity, to name a few.
In determining contributors to the higher CS rates in SJC, researchers identified additional missed opportunities for prevention, screening, and treatment in the Emergency Department (ED) and Correction Settings.
Knowing these missed opportunities, SJCPHS is working to improve identification and care coordination for pregnant mothers and infants with CS within the ED. SJC is also striving to improve case finding, education, and care coordination for patients of reproductive age and to improve referrals to other county and community programs that could provide additional patient resources.

Healthcare in Action (HIA) is a medical group that provides healthcare, substance use, behavioral health, and social services to older adults and other vulnerable individuals who are experiencing homelessness in Los Angeles County, which holds the highest population of unhoused individuals in the nation.
For HIA, streamlining care coordination and improving efficiency is critical to managing costs and providing the right care for this high-cost population. Since HIA is 100% mobile with no physically established medical clinics, having access to real-time longitudinal health records is an important part of their care model to understand patient history, provide trauma-informed care, and ensure proper post-discharge follow-up.
MX Access, MX’s longitudinal patient records, has been a valuable tool to the HIA case management team, providing medical histories at their fingertips, rather than having to wait for clinical documentation requests to be faxed and sent. MX Welcomes New Participants
Angeles Independent Physicians Association (IPA) is one of the fastest-growing IPAs in Los Angeles. Founded in 1995, Angeles-IPA, A Medical Corporation serves thousands of patients in the Los Angeles area each year through their network, which includes hundreds of family/general practitioners, pediatricians, and specialists. Their service area encompasses most of Los Angeles County and they are expanding into Orange County. They currently serve the communities of South Gate, Lynwood, Paramount, Downey, Huntington Park, Bell, Cudahy, Bellflower, and Long Beach.

Are You Ready for the January 2024 CalHHS Data Exchange Framework (DxF) Deadline?
2024 will undoubtedly be another year focused on advancing health and social services data sharing and connecting the California health ecosystem to transform care delivery in our state. We are already implementing CalAIM, which requires robust data sharing to achieve the goals of providing seamless care coordination for the most vulnerable of our Medi-Cal population. And next year, the state will ramp up its work in implementing the California Health and Human Services Agency (CalHHS) Data Exchange Framework (DxF), which requires most health organizations to sign a single statewide Data Sharing Agreement (DSA) and exchange health data with each other throughout California by January 2024.
These data sharing initiatives will help physician groups and practices provide better and more efficient care for their patients, spending less time and resources on faxing and chasing records, and more time on what’s really important—providing the best care. This is achieved by having secure and permitted access to health information on their patients, such as where they have been admitted, lab and test results, and discharge summaries. As the proverbial phrase goes, “Knowledge is power,” and physicians and care teams can make better and more timely care decisions with the right information.

Data is not the same as information. Physicians often tell us they want greater insight into their patients’ health care experiences, not necessarily more data – unfiltered records, often crowded with unhelpful “noise” – but deeper and more actionable insights that provide meaning for their medical practices and their patients’ health care journeys.
Physicians have shared stories over the years about how a simple alert sent by a hospital to the right medical practice, at the right time, letting the physician know the patient had been treated in the hospital, changed the course of their patient’s outcome. These notifications are a key tool in both improving care and earning significant value-based care savings. This is the power of admit, discharge, and transfer (ADT) notifications, real-time alerts from hospitals to a patient’s physicians and care coordinators.

Riverside Family Physicians (RFP) is a family practice dedicated to providing the highest quality of care for patients in the Inland Empire (Riverside and San Bernardino counties). RFP wanted their care coordination team to be able to follow up with patients after discharge without waiting for the patient to contact them. They joined the Manifest MedEx network in 2018 to access real-time health data to support whole person care before, during, and after a hospital visit.
More recently, RFP turned to Manifest MedEx to meet their data sharing requirements under the California Health and Human Services Agency’s (CalHHS) Data Exchange Framework (DxF). As a designated CalHHS DxF Qualified Health Information Organization (QHIO), MX is helping RFP meet their DxF requirements and securely participate in the DxF by exchanging data securely as required and permitted, reducing the burden on Riverside Family Physicians.
